Child & Family Connections

The Regional Office of Education #8 is the fiscal agent for Child & Family Connections #3. CFC#3 is one of 25 regional agencies that provide a single point of entry to the Illinois Early Intervention Program (EI). Early Intervention offers developmental support and case coordination services for families with children under 3 years old with developmental delays. Child & Family Connections employs credentialed Service Coordinators who assist referred families through the process of enrollment into the Illinois Early Intervention Program, as well as initiate referrals for evaluations and direct services. Eligible participants in the Early Intervention Program receive case coordination through CFC to monitor individualized goals, strategies, and services. Additionally, staff assist families with the transition from the Early Intervention Program into local school/care special education services based on the family’s priorities and the child’s needs.

Child and Family Connections also staffs a social emotional consultant to provide coordinators and providers information, training, and consultations through reflective supervision; a parent liaison to contribute an understanding of the EI program through the lens of a parent with special needs; and a coordinator who acts as a liaison to community partners, providers, and referral sources.

CFC#3 Our office provides services to families in Carroll, DeKalb, Jo Daviess, Lee, Stephenson & Whiteside counties. Anyone who suspects a child has delays can make a referral to CFC (parents, doctors, nurses, childcare providers, or friends). Simply call our office toll-free at: 888-297-1041 – or download a referral form. Completed referral forms can be faxed to: 815-554-4729.
