Regional Office of Education #8
CHILD & FAMILY CONNECTIONS
Child & Family Connections (CFC) is a family centered program for children birth–to-three who have or are likely to have delays in their development.
CFC serves as the single access point to Early Intervention services in the state of Illinois. There are 25 different offices located throughout Illinois. We are CFC #3. Our office provides services to families in Carroll, DeKalb, Jo Daviess, Lee, Stephenson & Whiteside counties. We have three locations: One each in Freeport, Sterling, and Sycamore.
Anyone who suspects a child has delays can make a referral to CFC (parents, doctors, nurses, childcare providers, or friends). You do not have to be certain there is a delay in development to refer a child. Simply call our office toll-free at: 888-297-1041. You may also click here for a referral form. Completed referral forms can be faxed to: 815-297-9032.
